Introduction Buruli ulcer is endemic in 30 countries worldwide, mainly localized in Sub-Saharan Africa around the Gulf of Guinea. To date, The Republic of Mali was not recognized as an endemic country for Buruli ulcer. Case presentation We report a case of non-healing skin ulceration of the left foot in a 15-year-old Malian patient acquired in Bamako and referred to France. Real time PCR for Mycobacterium ulcerans was positive in skin biopsy. The ulceration healed well after treatment with rifampicin and clarithromycin. Conclusion Emergence of Buruli ulcer in The Republic of Mali is of concern Present case urban origin raises questions about Buruli ulcer classical epidemiology.
